Please visit our school, by fax, by e-mail or this website to get the application form. The following supporting documents should be submitted with the completed form in-person or by post:

1. Hard copy of certificate of birth, 1 photo and 5 stamped self-addressed envelopes。 (Please write down the name and address of applicants and stick HKD$2 stamp on each envelope)
2. Application fee: HKD$40

• In-person: Please pay HKD$40 by cash with completed form.
• By post: Please post a crossed cheque payable to “EVI Services Limited” (Please write down the name and class of student with required documents with stick HKD$6 stamp)
